WHAT WE DO

End of Life Care​

End-of-Life Care provides comforting, non-medical support for individuals in the final stages of life, allowing them to spend their remaining time in the peace and familiarity of home. Our Care Partners work alongside hospice providers to offer physical, emotional, and practical assistance—helping to ease discomfort, maintain dignity, and support both the client and their loved ones during this deeply personal time. With sensitivity and compassion, we’re here to ensure no one has to face the end of life alone.

Our Care Partners can help with:

Comfort and Personal Care
  • Assisting with bathing, grooming, and changing clothes or bed linens
  • Repositioning in bed to prevent bedsores and reduce discomfort
  • Providing mouth care, skin care, and keeping the individual clean and dry
  • Monitoring for signs of discomfort or distress and notifying hospice staff or family
Nutrition and Hydration Support
  • Preparing soft or preferred foods (as tolerated)
  • Assisting with feeding or offering fluids
  • Gently encouraging hydration, if appropriate
Emotional and Companionship Support
  • Offering a calm, comforting presence
  • Sitting bedside to ensure no one feels alone
  • Listening, reading aloud, or playing soft music
  • Supporting spiritual or cultural preferences
Household and Environmental Support
  • Keeping the home quiet, peaceful, and clean
  • Light housekeeping (laundry, dishes, tidying common areas)
  • Creating a calm, comforting atmosphere with soft lighting and familiar items
Family Support
  • Providing respite for family Care Partners to rest or step away
  • Offering emotional reassurance and presence to loved ones
  • Communicating changes or concerns to family and hospice teams
Coordination with Hospice Team
  • Following care instructions from hospice nurses and staff
  • Being present during hospice visits to assist and support
  • Keeping records or notes as requested by the care team
