Alzheimer’s and Dementia Care provides specialized support for individuals living with memory loss, confusion, or cognitive decline. Our trained Care Partners offer patient, personalized care that focuses on safety, structure, and meaningful engagement—helping clients maintain their routines, independence, and dignity at home. With a deep understanding of the unique challenges of memory-related conditions, we’re here to support both clients and families every step of the way.
Our Care Partners can help with:
Cognitive and Emotional Support
- Gentle reminders for daily routines and tasks
- Engaging conversation to encourage connection and memory recall
- Redirection and reassurance during moments of confusion, anxiety, or agitation
- Structured daily routines to reduce stress and disorientation
Daily Living Assistance
- Help with bathing, dressing, grooming, and toileting
- Assistance with eating and hydration
- Support with walking and mobility to prevent falls
- Medication reminders and monitoring for adherence
Mental Stimulation and Activities
- Memory games, puzzles, and brain exercises
- Listening to music from the client’s era
- Reading aloud or looking through photo albums
- Gentle arts and crafts or familiar hobbies
- Sensory activities like folding towels or sorting objects
Home Environment Support
- Keeping the environment calm, clean, and clutter-free
- Ensuring safety (removing trip hazards, labeling rooms, etc.)
- Managing wandering risks (e.g., door alarms or supervision)
- Light housekeeping to maintain a safe living space
Companionship and Supervision
- Friendly presence and emotional companionship
- Accompaniment on walks or supervised outings
- Preventing loneliness, anxiety, or sundowning symptoms
- Monitoring behavioral changes and reporting concerns to the family
Family Support
- Regular updates on client condition and behavior
- Education for family members about dementia progression
- Respite care to allow family Care Partners time to rest and recharge
