Our Story

ApexCare is one of Northern California’s most trusted home care providers, helping thousands of families care for their loved ones since 1992. As an independent, locally-owned agency, we proudly serve 10 counties through our offices in Sacramento, Vacaville, Napa, Concord, and Stockton.

What sets us apart is our people. We’re a team of compassionate professionals driven by a shared mission: to be the brightest light in the lives of our clients. Each day, we strive to provide exceptional care that brings comfort, peace of mind, and dignity to our clients and their families. We understand the stress and uncertainty that can come with aging and caregiving — and we’re here to help every step of the way.

Jason Wu, President


Jason’s passion for senior care began more than 30 years ago, when his grandmother was diagnosed with Alzheimer’s disease. Witnessing the impact of her condition — and the vital role that loving, attentive caregivers played in her daily well-being — left a lasting impression on him. It was during that time he came to understand just how essential compassionate care is for families navigating the challenges of aging.

Since then, he has been driven by a deep sense of purpose to support the senior community with empathy, excellence, and integrity. Throughout his career, he has focused on building strong, service-driven organizations that prioritize people — both clients and team members — and uphold the highest standards of care. At ApexCare, he is honored to continue that mission by leading a team dedicated to bringing light, dignity, and peace of mind to the lives of those they serve.

Kathryn Fabiani, Director of Operations


Kathryn has always been a caregiver at heart — a role that came naturally as the eldest child in her family. Her journey into caregiving began early, deeply influenced by her relationship with her grandfather, who lived with Alzheimer’s disease throughout her childhood. Some of her most cherished memories were the moments he mistook her for her mother. In those tender conversations, she would ask about the grandmother she never had the chance to meet, and he would share stories that gave her a beautiful window into their lives.

As his condition progressed, so did his needs — and so did her desire to help. That calling led her to work as an EMT, a patient care technician in dialysis, and later in a behind-the-scenes role at a blood bank. Each experience deepened her understanding of what it truly means to care for others.

Sammi Ng, Finance & Accounting


With over 15 years of experience in administrative roles within the older adult community, Sammi has dedicated her career to supporting seniors and their families in meaningful and compassionate ways. Early in her journey, she developed a deep passion for senior care and found fulfillment in serving a population that deserves dignity, respect, and heartfelt attention.

In 2015, she joined ApexCare as the Accounting Manager, a position that has allowed her to grow professionally while deepening her connection to the senior care field. She takes great pride in her work and in upholding ApexCare’s mission — to be the brightest light in the lives of clients, their families, and the care partners who support them.

Raquel Nelepovitz, Human Resources


Raquel Nelepovitz joined ApexCare in 2024 as our Recruiting Manager, bringing with her a strong foundation in administration, finance, and client-facing roles. With years of experience managing office operations, accounting, and cross-functional communication, Raquel quickly became an invaluable part of our team. Her natural leadership, attention to detail, and people-first mindset led to her promotion as Human Resource Manager, where she now oversees recruitment, team development, and HR operations across the organization.

Raquel’s passion for supporting others is reflected in her diverse background — from managing accounts receivable at a growing firm to running her own photography business.. Her academic background includes a BS in Political Science from California State University, Chico, and a Master of Education from UC Davis.

Robin Mesman, Recruiting


Robin Mesman brings over 15 years of experience in caregiver recruitment and senior care staffing to her role as Recruiting Manager at ApexCare. Her deep understanding of the home care industry, combined with her people-focused approach, allows her to connect compassionate caregivers with the clients who need them most.

Before joining ApexCare, Robin held key recruiting and leadership roles at several senior care organizations, where she built strong community partnerships and developed successful hiring strategies. She is passionate about creating a welcoming and supportive experience for job seekers and ensuring ApexCare’s team reflects the heart and values of the care we provide.

Tammy Cahill, Business Development


Tammy Cahill brings over a decade of experience in senior living marketing, outreach, and client relations to ApexCare. Her career has been dedicated to helping families navigate the often overwhelming landscape of senior care with compassion, clarity, and expert guidance.

Before joining ApexCare, Tammy held leadership roles at several senior living communities and home care organizations, where she was instrumental in opening new territories, building strong referral networks, and facilitating dementia support groups. She is a Certified Dementia Practitioner and a passionate advocate for families facing the challenges of aging and memory loss. Tammy is known for her ability to build meaningful relationships within the community and develop outreach strategies that align with ApexCare’s mission — to be the brightest light in the lives of our clients.

Tricia Bracy, Compliance

Tricia Bracy joined ApexCare in 2018 as a Care Manager for our Vacaville office, bringing with her a deep passion for supporting seniors. Her journey in senior care began in 2010 after earning her CNA license, and she quickly discovered a calling in helping older adults maintain their independence and dignity. Tricia’s experience spans both facility-based and in-home care settings, giving her a well-rounded understanding of the industry.

In 2025, Tricia transitioned into a company-wide compliance role, where she now works closely with our care management teams to ensure operational excellence. From accurate payroll and billing to optimized scheduling, up-to-date certifications, and regulatory alignment, Tricia plays a key role in keeping our systems running smoothly and our care standards high. Her commitment to service and operational integrity makes her an invaluable part of the ApexCare team.

Diana Blaga, Branch Manager - Sacramento

Diana Blaga brings over 18 years of dedicated experience in the field of in-home care. What began as a hands-on caregiving role quickly grew into a lifelong calling—one that has shaped her into a compassionate leader and trusted expert in senior care. Diana has held a wide range of positions throughout her career, including care provider, supervisor, trainer, care consultant, marketing manager, and corporate branch manager. Each role has deepened her commitment to delivering high-quality, personalized care.

Since joining ApexCare in 2020, Diana has been an integral part of our Sacramento team. In 2025, her leadership, experience, and passion earned her a well-deserved promotion to Branch Manager. She takes pride in supporting both our clients and our team, always striving to exceed expectations and ensure that every family we serve receives the care and attention they deserve.

Debbie Kay, Branch Manager - Vacaville

Debbie Kay is the Branch Manager of ApexCare’s Vacaville office, where her leadership has helped make it one of the most respected and active home care providers in the region. She began as a Care Manager, but quickly found her passion in community outreach. As Community Relations Manager, she built strong relationships with families and partners that fueled the office’s growth.

Debbie brings extensive experience working with older adults. Before joining ApexCare, she was the Activities Director at La Mariposa Care and Rehab Center, coordinating programs for residents and families. She also spent six years as a Life Enrichment Coordinator at Paradise Valley Estates and over three years as Activity Director at Rancho Solano. These roles gave her deep insight into the needs of seniors and how to support them with compassion and care.

Adele Jeppesen, Branch Manager - Concord

Adele Jeppesen joined ApexCare in 2021 as a Care Manager in our Concord office, bringing with her a strong background in senior care and a deep personal passion for supporting older adults. In 2025, she was promoted to Branch Manager, where she now leads the Concord team with compassion, insight, and dedication.

Before joining ApexCare, Adele served as the Memory Care Manager at TreVista Senior Living in Antioch, where she also worked as a Life Enrichment Coach. These roles gave her extensive experience in working with individuals living with memory loss, as well as in designing meaningful engagement programs to enhance quality of life. Her professional background—combined with personal experiences—has shaped her exceptional ability to connect with clients and families, making her a valuable leader in our care management team.
